What is All-On-4, and how does it work?

Short answer

All-On-4 replaces a full upper or lower arch of teeth with one fixed bridge anchored on four dental implants. The back two implants are angled to use the bone that is already there. At ANEWU that bridge stays fixed in place rather than coming out for cleaning.

All-On-4 replaces every tooth in an arch with a single fixed bridge, carried by four implants rather than one implant for each missing tooth. The back two are usually tilted forward so they engage the denser bone nearer the front of the jaw. That angle is the entire idea, and it is why four implants can carry a full arch in jaws that could not take ten or twelve.

Because the bridge is screwed down rather than suction fitted, it does not come out at night and it does not rely on adhesive. ANEWU keeps full arch work in house, so the temporary bridge and the final bridge are both made in our on site lab instead of being sent away and waited on.

It helps to see the whole family before you settle on one. All of the full arch implant options are planned from the same 3D scan, so the scan comes first and the decision comes after.

Replacing missing teeth

How do I replace a full arch of missing teeth?

A full arch of missing teeth is replaced three ways at ANEWU: a fixed bridge on four implants, the same bridge on six implants for more support, or a removable overdenture that snaps onto a smaller number of implants. A 3D scan at your consultation shows which your bone supports.

If you have ever wondered how to replace missing teeth without wearing something that comes out every night, this is the short version. The more implants your bone can carry, the more fixed and the more stable the result, and the scan is what decides the number.

Four implants carry a fixed bridge across the arch. Six spread that same load a little further and add a margin if one site heals slowly, which is what All-On-6 is for.

If your bone is limited, or a removable result simply suits you better, mini implant dentures snap onto a smaller number of narrower implants instead.

With any of the fixed options, same-day temporary teeth are possible for qualifying cases, so you are not asked to spend the healing period without teeth.

Honest limitation. Not every jaw has the bone volume for four implants, and that is a measurement rather than a judgement call. The scan settles it in a single appointment, and there is usually a route forward either way.
Bone loss

What are my options if I have been told I do not have enough bone?

If another office told you that you lack the bone for implants, two routes usually remain. Bone grafting rebuilds the jaw so implants can be placed later. Zygomatic implants anchor in the cheekbone instead, which can avoid grafting entirely. Dr. Abtin uses a 3D scan to determine which applies.

If you have ever wondered why one surgeon says no and another says yes, this is usually the reason. The upper back jaw loses height quickly once the molars are gone, so what matters is where the bone is missing, not only how much of it.

Bone grafting adds that height back over a healing period, after which implants can be placed in the usual way.

Zygomatic implants take a different route, anchoring in the cheekbone above the sinus so that a graft can often be skipped.

From your first scan to your final fixed bridge

Five stages. Most of them are shorter than people expect, and the two that matter most happen on the same day.

    Consultation and 3D imaging

    Your jaw is scanned in three dimensions so bone height, bone width and the way your bite closes can all be measured rather than estimated. What to expect at your first visit is set out step by step.

  2. 02

    Your treatment plan

    The exact position and angle of all four implants is mapped before surgery day, using 3D imaging and guided surgery to plan around the bone you actually have.

  3. 03

    Implant placement

    Any remaining teeth in the arch are removed and the four implants are placed in a single outpatient visit at your chosen office.

  4. 04

    Same-day temporary teeth

    For qualifying cases a temporary fixed bridge is attached that same day. Same-day teeth are available for qualifying cases, not for all of them.

  5. 05

    Healing and your final bridge

    After a healing period the final bridge is made in our on site lab and fitted. Your post-op instructions cover the weeks in between.

Cost and financing

How much do All-On-4 implants cost in Atlanta?

All-On-4 cost at ANEWU depends on how many implants your case needs, whether bone grafting comes first, and which materials your final bridge uses. Dental insurance typically does not cover implant treatment. ANEWU accepts CareCredit and Proceed Finance and can arrange a monthly payment plan before your procedure.

Dental insurance and implant treatment

Most dental plans were written around fillings, crowns and dentures, so implant treatment usually sits outside what they cover. A plan with an annual maximum may still contribute toward extractions or toward the restoration itself. Our team will read your policy with you rather than guess at it.

CareCredit and Proceed Finance

Where is this treatment performed?

Implant surgery is performed at all three ANEWU offices in Georgia. Patients north of Atlanta most often use the Cumming or Dawsonville offices, both just off the GA-400 corridor, while the Buckhead office serves patients inside the perimeter. Records move between offices, so follow up can happen at whichever address is closer.

For qualifying cases, yes. A temporary fixed bridge can be attached the day of implant placement, so you're not without teeth while your final restoration is made.

No. The bridge is fixed onto the implants and stays in place, so it is not taken out at night the way a denture is. You clean around it daily, much like natural teeth, and our team will show you the technique at your follow-up.
